Foundation Stock horses MUST have this form completed by a licensed DVM, if this form is NOT complete your certificate will read "UNVERIFIED"
This form is for BOTH Premium and Foundation Stock division, you may also use this form if you are doing additional DNA or genetic testing.
This form is for horses that have been color tested or that has requested color testing, The coat color will be printed as "VERIFIED" on the certificate.
This form is transferring ownership from seller to buyer.
This show record must be turned into EOA by the end of each quarter to qualify for "High Point" award.
